CTF Illinois is seeking a dedicated and organized QIDP / Case Manager to support individuals with developmental disabilities in our Community Day Services Program at our Tinley Park location. Position Overview

  • Schedule: Monday–Friday (daytime hours) with a flexible, hybrid schedule (in-person and remote)
  • Develop and implement individualized strategies based on each person’s goals, preferences, and personal plan
  • Coordinate services by maintaining regular contact with individuals on your caseload
  • Participate in discovery and interdisciplinary team meetings
  • Complete required assessments, documentation, and consents
  • Monitor progress and outcomes through monthly reviews and reporting

Qualifications

  • Bachelor’s degree in a human services-related field (required)
  • Must meet certification and training requirements for QIDP
  • At least 1 year of experience working with individuals with developmental disabilities
  • Valid driver’s license with an acceptable driving record
  • Strong organizational skills and ability to work independently
  • QMRP/QIDP certification preferred
  • Previous DSP training a plus
