Case Manager (QIDP)/ SDA

CTF Illinois is an essential employer seeking a Case Manager (QIDP)/SDA to support our Home Based Services individuals with developmental disabilities. If you have experience working with individuals with developmental disabilities, are highly organized, and are able to work independently, we have the job for you!
Overview:

  • Work schedule: Monday through Friday generally
  • Submitting DHS required documentation in a timely manner
  • Conduct home visits regularly
  • Coordinate and Implement an Individual Service Plan (ISP) for individuals receiving services by following the recommendations made by medical and professional consultants
  • Assist PSW with implementing the ISP, completing budgeting, shopping and other money management issues
  • Coordinate with other departments including, but not limited to: Nursing, CILA, and Day Program
  • Monitor each individual’s progress by completely monthly reviews and maintaining each individual’s personal history book
  • As required by DHS, complete an additional 12 hours of continuing education units per year, at least 6 of which will be provided from outside the agency
Requirements:
  • Bachelor Degree from a four-year college or university in a human service related field
  • Meet requirements for certification and training as a QIDP
  • Minimum 1-year experience working with individuals with developmental disabilities
  • Valid driver’s license with acceptable driving record per company requirements
  • Ability to work independently and prioritize work in timely manner
  • Certification and training as QMRP/QIDP preferred
  • Previous DSP training preferred
