Case Manager (QIDP)

CTF Illinois is seeking a Qualified Intellectual Disability Professional (QIDP)/ Case Manager to support people with developmental disabilities at our Community Day Service Program in Orland Park, IL. If you have experience working with people with developmental disabilities, are highly organized, creative, and motivated, we have the job for you!

Overview:

  • Work Schedule:  Monday through Friday, Days
  • Develop Implementation Strategies with each person based on their choices and goals within their personal plan.
  • Assist in coordinating services by meeting regularly with people on your caseload, including attending discovery meetings and inter-disciplinary meetings.
  • Complete required consents and assessments for each person on your caseload
  • Monitor each person's progress and choices by completing monthly reviews 

Requirements:

  • Bachelor Degree from a four-year college or university in a human service related field
  • Meet requirements for certification and training as a QIDP
  • Minimum 1-year experience working with individuals with developmental disabilities
  • Valid driver’s license with acceptable driving record per company requirements
  • Ability to work independently and prioritize work in timely manner
  • Certification and training as QMRP/QIDP preferred
  • Previous DSP training preferred
